Responsibilities Coordinate and oversee individualized care plans for residents.
Serve as the primary point of contact for residents, families, and healthcare providers.
Conduct assessments to ensure residents' needs are met and adjust care plans as necessary.
Advocate for residents' well-being, ensuring their physical, emotional, and social needs are addressed.
Maintain accurate documentation and comply with all regulatory requirements.
Collaborate with staff to ensure seamless delivery of services.
Qualifications Bachelors degree in social work, Nursing, or a related field (preferred).
Experience in case management, senior living, or healthcare settings.
Strong communication, organizational, and problem-solving skills.
Empathy, patience, and a genuine passion for working with seniors.
Knowledge of state and federal regulations related to assisted living.
