Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We offer more than 180 integrated programs in Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.
Beach 21 is a building operated by Community Builders. Beach 21 is a residential building in the Far Rockaway neighborhood of Queens. The project consists of 23 residential units, 5 singles and 18 families with CAMBA, Inc. providing the on-site services.
Position: Case Manager
Reports To: Program Manager
What The Case Manager Does:
Maintain professional relationships with clients and client confidentiality
Practice Universal Precautions/Standard Protocol & Procedures
Comply with Federal, State, City and CAMBA security and privacy policies intended to protect the security and privacy of individually identifiable health information
Review documentation establishing tenants' eligibility for program and make file copies
Create and maintain tenant files
Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments
Collaborate with tenants to prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals
Assist tenants in locating community resources and referrals to appropriate services within and outside CAMBA
Work with tenants to overcome barriers to goals and to advocate for themselves toward self-sufficiency
Recommend and implement strategies to encourage tenant participation
Monitor progress toward goals via telephone contact and/or face-to-face visits; document via progress notes
Participate in case conferences
Follow-up with tenants and with referral organizations regarding contact and progress
Provide required information for weekly/monthly/quarterly/annual reports
May recommend closing of cases when goals are achieved and stability is maintained, or when participation is unlikely or eligibility changes
Act as tenant liaison/advocate with outside organizations on education, healthcare, housing, legal issues, etc.
Schedule appointments for tenants with referral organizations
Escort tenants to appointments (educational, medical, social service, etc.)
Assist tenants in completing applications for benefits and entitlements; may process applications on tenants’ behalf
Follow-up with tenants after successful completion of primary goals to ensure stability
Evaluate actual living conditions of tenants through home visits
May prepare marketing materials for the program
May prescreen tenants over the telephone for eligibility and schedule intake appointments
Input tenant data and progress information into an automated database
Minimum Education/Experience Required:
Experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience
Other Requirements:
Must be fingerprinted and obtain a Criminal History Check (CHC) with the Justice Center for the Protection of Vulnerable People with Special Needs; must maintain fingerprint clearance throughout employment
As a Mandated Reporter, you must immediately report suspected abuse or neglect to the Vulnerable Persons Central Register (VPCR) Hotline at 1-855-373-2122 and complete a CAMBA Incident Report
Flexible hours may be needed occasionally to provide shift coverage until 8 pm
Compensation : 40,000-46,250 annually. Salary ranges are the low and high end for the applicable position and program. The salary offered will be based on factors such as experience, education, budget and program size, internal equity, skills and other required factors.
Status: Full-time (35 hours per week)
