A New Leaf, a nonprofit dedicated to providing critical resources and safe housing for individuals and families in crisis, is seeking a compassionate and skilled Case Manager to join our Domestic Violence Support team.

Hourly Pay Rate : $20.00

Location : Apache Junction, AZ

Schedule: Sunday - Wednesday; 11am-9pm *Required to complete our "New Hire Orientation" held on the first work week of employment, Monday-Friday, 8:30am-5pm; subject to change upon completion of orientation requirements which may grant early release of orientation before Friday; orientation not offered evenings or weekends.

QUALIFICATIONS: Education :

High School Diploma or GED, required.

Associate or Bachelor’s Degree, preferred.

Bilingual verbally in Spanish, preferred.

Experience:

Experience working with domestic violence, homeless or economically disadvantaged population required.

Knowledge of community resources.

Knowledge of and/or experience with trauma informed practices required.

Primary Responsibilities:

Role model appropriate behaviors to callers on the hotline and clients in the shelter, at all times, utilizing trauma informed principles and practices.

Provide case management, crisis intervention, domestic violence education, and resources and referral services to callers/clients on the crisis hotline and in the shelter.

Be responsible for issues relating to admission and discharge, in conjunction with the programs definition and policies as relevant to program/client needs.

Perform screenings, lethality assessments, and basic needs assessments for potential clients according to procedure. Assist with the introduction of program and facility orientation to incoming clients. Assist with client intake/exit paperwork as needed.

Co-develop 24/7 Safety/Crisis Plans with every household, depending on the safety needs and risk factors identified by the client.

Monitor the implementation of a strength-based service plan and progress of household towards goals.

Assist households in obtaining community services and benefits, securing appointments with outside services and agencies as needed; including, identification of funding sources and linkages to other agencies/services.

Provide referrals and/or coordinating transportation for households to community services as relevant to program/client needs.

Coordinate client satisfaction surveys with individual clients on a regular basis.

Facilitate and coordinate discharge planning transition and follow up according to plan.

Compliance/Requirements

Valid Driver’s License with reliable transportation.

Current Auto Liability Insurance.

Class 1 Fingerprint Clearance Card.

39 Month Motor Vehicle Record.
