Job Title and Details Case Manager, FTE: 1.0 (40 hours/week). Starting Pay: $24.26 - $29.62/hour.

Compensation Note Candidates who are highly proficient in English and Spanish may be eligible for a 5% bilingual differential pay (certified by examination).
Purpose Of Position Individualized case management is a core component of Sponsors' programs. Case managers conduct assessments, initiate program referrals, develop dynamic and supportive case plans, and carry out overall operations of the Transitional Housing program. The Case Manager will learn and reflect Sponsor’s values by integrating behaviors and best practices into daily work.
Position Reporting This position reports to the Transitions Program Director. This position does not have supervisory responsibilities.
Job Responsibilities Oversee all aspects of Case Management for assigned program participants
Perform screening and intake for incoming residents using program-approved assessment tools
Monitor status of individualized case plans based on validated risk assessments and participant goals
Initiate referrals; coordinate and communicate regularly with participants’ parole officers and treatment providers
Monitor compliance with conditions of each participant’s release and all Sponsors program requirements
Monitor compliance with requirements of other related programs, including cognitive groups, drug/alcohol treatment, parenting classes, and others as applicable
Conduct regularly scheduled 1:1 sessions with each participant and maintain communication as necessary
Perform scheduled and random drug and alcohol screenings
Utilize web-based programs and other systems to chart participant progress, maintain accurate entry/discharge records, and coordinate care
Program Compliance and Transition Ensure program participants are compliant and progressing appropriately in transition/exit planning, including development of transition plans with a goal toward long-term stable housing and approval of assigned parole officers
Monitor and discuss participants’ employment, savings, and financial management, particularly as it relates to their transition from Sponsors into stable housing
Maintain appropriate participant records, electronic and physical, as required by Sponsors, collaborating agencies, and grant requirements
Operations and Collaboration Assist in the coordination of program operations
Monitor common areas and housing units to ensure safety, security, and cleanliness
Receive and route calls, faxes, and walk-in program visitors
Participate in Transitions and Sponsors all-staff meetings and trainings
Maintain regular written and verbal communication with Sponsors staff and community partners regarding participant progress and conduct
On a rotating basis, carry an agency cell phone after hours to provide guidance to Support Staff and respond to emergencies
Other Duties Perform other related duties as assigned by the Transitions Program Director

Education and Experience 2+ years of experience providing direct client services; OR
1 year providing social services to individuals in the criminal legal system
Preference for case management experience
Higher education in a related field preferred (but not required)
Work Environment The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed
This position may travel between locations at the direction of the Transitions Program Director
May encounter frequent interruptions and individuals experiencing distress
Physical Demands Walking up and down stairs
Sitting at a desk for prolonged periods
Office environment noise is typical
Post-conviction Supervision Status Successful candidates must not be on parole or probation and cannot be in a diversion program. Employment is contingent on a background clearance by the Oregon Youth Authority through their external review and approval process.
