Role Description
This is a full‑time on‑site role for a RN/LPN Case Manager located in Eugene, OR. The RN/LPN Case Manager will be responsible for managing patient cases, coordinating care plans, and collaborating with healthcare providers to ensure seamless patient care. Day‑to‑day tasks include assessing patient needs, developing and implementing care plans, monitoring patient progress, and documenting medical information. The role also involves educating patients and families about treatment options and available resources.
Qualifications
RN or LPN licensure in Oregon and Case Management experience
Minimum of 2 years of clinical experience, ideally in behavioral health, psychiatric care, or integrated care settings (training provided)
Ability to work collaboratively with healthcare providers
Excellent assessment and care planning skills
Experience with patient education and advocacy
Experience working with adults and/or diverse populations preferred
Strong communication, teamwork, and organizational skills
Compassionate, patient, and motivated to support clients’ long‑term recovery and well‑being
Comfortable with technology and EHR systems; willing to learn AI‑assisted documentation tools
