Job Title: Case Manager

Location: Morristown, NJ 07960

Start Date: November 24, 2025

Estimated End Date: February 21, 2026

Overview:
The Case Manager will be responsible for coordinating and managing patient care throughout the acute care hospitalization process. This role involves comprehensive discharge planning, transition management, utilization review, and quality assurance to ensure efficient and effective patient outcomes.

Key Responsibilities:

Coordinate and oversee patient care during acute hospitalization.

Manage all aspects of discharge planning and transition of care.

Conduct utilization management reviews to ensure appropriate levels of care.

Perform quality screenings and assessments in compliance with hospital and regulatory standards.

Collaborate with interdisciplinary teams including physicians, nursing staff, social workers, and external partners.

Advocate for patients’ needs to promote optimal outcomes and continuity of care.

Maintain accurate and timely documentation in accordance with hospital policy and standards.

Required Skills & Experience:

Minimum of 2 years of acute care experience (strongly preferred).

Inpatient Case Management experience is required .

Knowledge of case management, discharge planning, and managed care processes.

Strong interpersonal, organizational, and communication skills.

Ability to multitask and collaborate effectively in a fast-paced environment.

Education & Licensure:

Bachelor of Science in Nursing (BSN) – required

Current RN license – required

Work Type: Onsite
