About the Program and Case Manager Position:
Our Transitional Foster Care Program for Unaccompanied Children (UC), overseen by the Office of Refugee Resettlement (ORR), ensures the well-being and safety of unaccompanied children (ages 0-17) who cross the US border without an adult. Unlike traditional congregate care settings, our approach involves placing unaccompanied children in foster homes while diligently searching for and assessing potential sponsors identified in the US. During their stay with us, we provide medical care, case management, engaging life skills classes, and essential mental health services.
The Case Manager acts as the centralized point of contact for all UC in ORR custody who have been placed in Cayuga Centers’ Transitional Foster Care (TFC). The primary role of the Case Manager is to ensure that each UC is safe and cared for, and is reunited with a suitable parent, guardian, or sponsor in the most expeditious manner possible.
How you will get to use your skills as a Case Manager….
Ensure safety for all youth within each foster home
Manage a high turnover caseload depending on incoming placements and releases
Coordinate initial intake for each youth
Manage reunification process for each youth
Keep information updated in the UC Portal
Coordinate and handle an estimated case load of 8 - 12 youth at any given time.
Schedule and confirm each youth has an initial medical evaluation within forty-eight hours of arrival with the Family Nurse Practitioner
Coordinate and assist the clinician to ensure the admission assessment and family tree are completed within three days of arrival.
Coordinate and assist clinician to ensure the psychosocial summary and Individualized Service Plan (ISP) are completed within seven days of arrival.
Ensure all assessments, Significant Incident Reports (SIR), team case notes, and foster family daily reports are current in the hardcopy file, electronic file, and UC Portal.
Coordinate each case with the clinician and GDIT Case Coordinator to review progress, concerns, and make recommendations on release.
Coordinate the discharge and release of child to their sponsor
Ensure each youth is enrolled and attending school
Respond to client emergencies within or outside work hours
Responsible for Chart Audits
Must be willing and able to positively step forward to assist in other programs, if requested, to maintain a full caseload, assist fellow staff/families, and complete other projects and duties as assigned by your immediate supervisor, program leadership, or Chief Executive Officer.
Required Experience of Case Manager:
Bachelor’s degree in relevant behavioral/social sciences field required, Master's preferred
One to three years of progressive experience in case management preferred
Child welfare experience and professional licensure - strongly preferred
Ability to work in a fast-paced environment
Strong skills in Microsoft Office including Word, Excel, PowerPoint, and Outlook applications
Ability to work well with people from many cultures including recent immigrants, non-English speakers, and LGBTQ children
Bilingual - English & Haitian Creole or English & Spanish required. All agency documents and records are in English, so English writing proficiency is required.
Valid NYS Driver's License preferred
Physical Requirements of a Case Manager:
Computer work requires sitting at a desk for long periods of time
Must be able to lift up to 30 pounds
Time driving and/or taking public transportation to conduct client and family visits
Regular hours vary depending on program needs; Must be flexible to meet program needs
Must be available to work afternoons, evenings and weekends required
