Join the Arizona Department of Economic Security (DES) as a CASE MANAGER 2 within the Division of Developmental Disabilities (DDD). This role supports individuals with developmental disabilities and their families by providing high‑quality, client‑centered services that promote independence and community participation.

Job Summary
Described above. The role is a critical part of the DES mission to strengthen individuals, families, and communities for a better quality of life. The CASE MANAGER 2 will work independently and collaboratively to facilitate service plans, conduct field visits, and maintain thorough case records.

Job Duties

Drive to conduct onsite visits to members’ homes, schools, and program sites to build relationships and discuss service needs and supports.

Develop, review, monitor, and evaluate Person‑Centered Service Plans (PCSP) in accordance with mandated timeliness and document individual progress, concerns, and referral appropriate services.

Facilitate meetings and program visits to assist with member/family choices; complete referrals for assessed/covered services.

Collaborate with community agencies to provide assistance and support to members and families in crisis.

Write comprehensive progress notes and maintain case records; prepare reports and special projects as required.

Knowledge

Health Insurance Portability and Accountability Act (HIPAA).

Medical and behavioral health issues and terminology.

Methods, procedures, and techniques used for PCSP development.

Federal and State statutes and agency policies relating to developmental disability care.

Principles and practices of case management.

Skill

Developing, evaluating, and maintaining member service plans.

Interviewing, counseling, and interacting with members and families.

Problem identification and resolution.

Ability

Work independently and within diverse work teams.

Organize and manage case loads.

Document case files and information within required timelines.

Maintain confidentiality.

Drive a motor vehicle.

Selective Preference

Bilingual proficiency in Spanish/English.

Pre‑Employment Requirements

Candidate must be an Arizona licensed Registered Nurse in good standing OR possess a Bachelor’s Degree in Social Work, Psychology, Special Education, or Counseling AND have at least one year of experience providing case management to persons who are elderly and/or have physical or developmental disabilities and/or diagnosed with a Serious Mental Illness (SMI); OR minimum of two years experience working with the same populations.

A Bachelor's Degree in Human Service, Behavioral Science, or a related field AND at least one year of relevant case management experience OR at least three years experience working with individuals with intellectual or developmental disabilities.

Valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41‑1968.

Background checks of the Department of Child Safety Central Registry and Adult Protective Services Registry.

Successful completion of background and reference checks; employment is contingent upon required clearances.

Driver’s license required if job involves vehicle use.

Completion of E‑Verify employment eligibility verification.
