What You’ll Do
The Case Manager is responsible for developing and monitoring comprehensive participant service plans in collaboration with assigned participants. This includes referrals to internal and external services to stabilize medical and mental health conditions, barrier removal, job readiness coaching, job placement assistance, and retention services.
Your Responsibilities
Initiate services for participants following their clinical assessment, providing orientation to services, explaining their rights and responsibilities, and answering questions.
Develop and monitor wellness plans for participants with untreated or unstable medical and/or mental health conditions that adversely affect employability, coordinating referrals to community-based providers.
Regularly meet with assigned participants to monitor progress and compliance with wellness plans, and assist with mitigating barriers to employment (housing, domestic violence, childcare, transportation, reasonable accommodations).
Conduct regular phone follow‑up appointments to assess progress toward wellness plan goals.
Outreach to participants who have failed to meet mandatory appointments and other program requirements to re‑engage them with wellness services.
Communicate with external medical and mental health providers to ensure participant compliance and timely completion of wellness plans.
Provide intensive individual coaching for participants in job search activities and assist with work‑related and transitional benefits issues.
Participate in case conferences with other WeCARE service providers as needed.
Document participant services and activities in Fedcap and HRA management information systems (e.g., SEAMS, FedcapCARES).
Qualifications
Bachelor’s degree in social work or related field (psychology, vocational rehabilitation, occupational rehabilitation). MSW or master’s degree preferred.
One (1) year of case management or related experience.
Bilingual (English/Spanish) preferred.
Compensation
$23.08–$24.04 per hour.
