Job Description
The Care Manager Specialist has overall responsibility and accountability for coordinating all aspects of assigned health home member's care. The Care Manager will track and arrange appointments, educate members, and coordinate other aspects of the member's health and community services. Responsibilities include but are not limited to:
Assisting members with consent forms
Conducting outreach and engagement activities
Conducting initial and periodic needs assessments
Assisting with the development and execution of care plans
Providing links to services and communicating among care management team, providers, members, and families caregivers
Minimum Qualifications:
Must have a minimum of two years' experience in Healthcare, Managed Care setting, or related field
Ability to perform in a busy, changing, multi-tasking work environment
Excellent written and verbal communication skills
Excellent interpersonal skills
Excellent organizational and documentation skills
Proficient in Microsoft applications
Community Based Work: Will require traveling within borough
Education: Bachelor degree required; master degree preferred in related mental health field
Location: Staten Island, NY
Pay: $53K
