Job Description Responsible for the assessment, reassessment, care planning, and coordination of care and services. Includes ongoing monitoring of an appropriate and effective person-centered care plan, member education, and care management. Regularly communicates with the member's PCP and other providers, and integrates feedback from the member, caregiver, and other providers into the assessment and planning.
Qualifications Valid NYS RN License
2+ years’ experience in Behavioral Health
Bachelor’s Degree

The candidate will oversee the care of members transitioning to the community.
Assigned to 1-2 transitional homes located in Westchester, Manhattan, Queens, Bronx, and Brooklyn.
Caseload of approximately 125 members. Most Care Managers start between 9 am and 10 am, with flexible start times.
Mileage reimbursed at $.535/mile. Mileage calculation starts from the first location to the next, not from the candidate's home.
Clinical experience in managed care, including disease or case management, is required. Experience in utilization review, concurrent review, and risk management is a plus.
Understanding and application of Care Management principles and Person-Centered Service Planning.
Ability to apply Milliman Care Guidelines and other clinical guidelines. Knowledge of coverage policies, benefit limitations, and disease processes for chronic illnesses such as depression, dementia, and Alzheimer's disease.
