Overview
Care Manager - (Mandarin/Cantonese) at Care Design New York. The role helps people with intellectual and/or development disabilities identify and realize their long-term and short-term goals by developing, implementing, and monitoring person-centered Life Plans. This requires strong relational and organizational skills and commitment to delivering high-quality service. The position is hybrid, with required visits to members and their families, in-office trainings, and some work from home. If supporting members outside New York City, a driver’s license, reliable transportation, and appropriate insurance are required.

Responsibilities

Advocate for and with members to ensure informed decision making, informed consent, and appropriate guardianship.

Schedule, lead, and collaborate with members and the interdisciplinary team to develop a comprehensive, person-centered Life Plan that reflects needs and life goals.

Implement, update, and monitor Life Plans; facilitate Life Plan reviews and approval processes.

Ensure integration of needed and preferred supports and services (medical, behavioral, social, habilitation, dental, psychosocial, community-based, and long-term facility-based supports).

Maintain ongoing contact with key people in a member’s life.

Ensure timely submission of documentation (Life Plan, progress notes, etc.).

Assist members with maintaining benefits such as Social Security, SSI, Medicaid and Medicare, and Food Stamps.

Help members resolve living problems such as housing, utilities, the judicial system, and general safety.

Report abuse or neglect immediately when observed or reported.

Qualifications

Bachelor’s degree with 2+ years of relevant experience, OR

License as a Registered Nurse with 2+ years of relevant experience, OR

Master’s degree with 1+ year of relevant experience.

Strong communication skills (verbal and written) and strong interpersonal and organizational abilities.
