Job Overview The Care Manager/"Designated Care Manager" is responsible for providing high-quality care and services to residents and their families in assisted living/long-term care and reminiscence neighborhoods. This role involves hands-on care, emotional and physical support as outlined in each resident’s Individualized Service Plan (ISP), and maintaining a safe, home-like environment. The Care Manager demonstrates Sunrise’s mission to champion quality of life for seniors, in accordance with all relevant laws, regulations, policies, and standards.
Responsibilities & Qualifications Essential Duties Supporting our Mission, Principles of Service, and Core Values, the Care Manager’s responsibilities include:
Reviewing, documenting, and initialing the Daily Log for resident information.
Participating in ISP development and updates.
Managing designated residents during shifts, ensuring their safety and well-being.
Monitoring and reporting changes in residents’ conditions.
Responding promptly to security system and call bells.
Greeting visitors and responding to calls professionally.
Assisting with continence management and maintaining sanitation standards.
Communicating with families and acting as a resource.
Supporting residents’ independence, dignity, and participation in activities.
Engaging residents in life skills and enrichment activities.
Maintaining safety regulations and practicing good hygiene and safety procedures.
Risk Management & Safety Ensuring compliance with occupational health and safety regulations.
Practicing safety procedures, reporting incidents, and hazards.
Handling hazardous materials safely and reporting exposures.
Housekeeping & Laundry Maintaining clean resident rooms and common areas.
Washing resident laundry and linens as needed.
Dining Service Serving meals and assisting in dining room operations.
Following dietary restrictions and observing residents’ dining habits.
Reminiscence Neighborhood Care Supporting residents with life skills and social activities.
Participating in community engagement and communication with families.
Team and Training Participating in team meetings and training sessions.
Complying with infection control and safety policies.
Core Competencies & Qualifications Ability to stay calm and handle multiple priorities.
Effective communication skills.
Organizational and decision-making skills.
High School diploma or GED; CPR/First Aid as required.
Experience with seniors preferred.
