Job Description The Care Manager/“Designated Care Manager” is responsible for providing the highest quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands-on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, home-like environment. The role promotes the Mission for Sunrise Senior Living: “to champion quality of life for all seniors” in accordance with laws, Sunrise standards, policies, and procedures to deliver high-quality care and services.
Responsibilities Essential Duties Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents.
Participate in the development of the ISPs and monthly updates.
Review designated assignments.
Be responsible for a designated group of residents during the shift, know where residents are, and physically check on them throughout the shift.
Observe, report, and document symptoms and conditions of residents for changes in condition (e.g., skin, behavior, alertness, weight, diet, and participation in activities).
Attend daily Cross Over meetings led by the care manager.
Notify supervisor and resident care director if a resident has increased care needs.
Inform supervisor of any resident changes in condition.
Respond to security system and resident call bells promptly and take appropriate action, including resetting call bells.
Greet guests, family members, residents, and team members; answer, direct, and respond to calls in a timely, courteous, and professional manner.
Assist with continence management and dispose of continence products properly to ensure sanitation of resident suites and restrooms following standard procedures.
Communicate with families and act as a resource as needed.
Help residents maintain independence and physical safety while promoting dignity, adhering to Resident Rights and Sunrise Principles of Service.
Participate/lead in activities of daily living (ADLs) and invite, encourage, and assist (IEA) residents to participate in ISP-indicated activities.
Engage residents in life skills and enrichment activities throughout the day in reminiscence.
Respond with empathy to each resident, considering individual communication patterns and basic human needs.
Maintain a warm, personal, and positive environment and ensure safety regulations are followed.
Practice standard care precautions for cleanliness, hygiene, and health.
Host and engage in activities with residents daily.
Risk Management & General Safety Partner with the community team to ensure compliance with regulations, promote risk management, and adhere to safety rules and regulations.
Follow safety procedures including PPE, fire extinguishers, SDS, and lockout/tagout.
Report accidents/incidents immediately and report unsafe or hazardous conditions/equipment.
Keep cords, carts, equipment, and hazards out of the way and in compliance with fire codes.
Understand proper methods for attending to and disposing of bloodborne pathogens, bodily fluids, infectious waste, sharp objects, and hazardous materials.
Report occupational exposures immediately.
Ensure oxygen tanks are stored safely and maintained at prescribed levels.
Housekeeping & Laundry Maintain and clean resident rooms, including making beds, trash removal, clothing organization, spot cleaning, and mopping as needed.
Wash resident laundry as ISP specifies; wash and fold dining room linens and napkins.
Complete assigned housekeeping tasks and keep common areas clean.
Dining Service Serve meals in the dining room and assist with dining room operations as assigned.
Promote a pleasant dining experience and follow dietary indications on ISP/Confidential Diet Board.
Observe and document changes in dining habits in the daily log; provide room service as needed.
Practice safe food handling in line with care precautions.
Reminiscence Neighborhood – Specific Responsibilities IEA residents in life skills and enrichment activities as indicated on ISP and demographic profile.
Encourage participation in afternoon social and daily routines with life skills integration.
Incorporate “Creating Pleasant Days” into daily routines and blend various multi-sensory experiences.
Participate in monthly letters home and family correspondence with residents.
Training & Team Contribution Work as part of the team toward shared goals; model the Team Member Credo.
Serve residents and guests through Principles of Service; participate in engagement surveys and improvement workshops.
Attend regular meetings and training; follow infection control procedures and state/provincial requirements.
Maintain required trainings to meet Sunrise standards and perform other duties as assigned.
Core Competencies Remain calm in difficult situations; handle multiple priorities.
Strong written and verbal communication; good organizational and time management skills.
Experience & Qualifications High School diploma/GED (as required by regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years old; experience with seniors preferred.
Ability to make decisions in the resident’s best interest.
