Overview Care Manager role at Sunrise Senior Living. Job Description The Care Manager/“Designated Care Manager” is responsible for hands-on care and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, home-like environment.
Responsibilities Resident Care: document and review Daily Log, participate in ISP development and updates, manage a designated group of residents during the shift, observe and document changes in residents, attend daily cross over meetings, notify supervisors of changes, respond to call bells, greet guests and residents, assist with continence management, communicate with families, support resident independence and safety, assist with ADLs and reminiscence activities, foster empathy and respect for resident rights, and maintain a warm, calm environment that follows safety regulations and infection control procedures.
Risk Management and General Safety: ensure compliance with health and safety regulations, use PPE and safety procedures, report accidents and hazards, keep exits clear, and handle potentially hazardous materials safely.
Housekeeping and Laundry: maintain resident rooms and common areas, handle resident laundry, and manage dining room linens and tasks.
Dining Service: serve and support meals, ensure a pleasant dining experience, follow dietary requirements, and monitor dining changes.
Reminiscence Neighborhood Care: enable engagement in life skills and daily routines, promote creating pleasant days, and incorporate residents’ life skills into daily activities.
Training and Team Contribution: participate as a team member, uphold Sunrise Credo and Principles of Service, contribute to engagement programs, attend meetings and training, comply with infection control and training requirements, and perform other duties as assigned.
Core Competencies: manage multiple priorities, communicate effectively, maintain organization and time management, exercise good judgment and problem-solving.
Experience and Qualifications High School diploma/GED (as required by state/provincial regulations)
CPR and First Aid as required by regulations
Must be at least 18 years of age
Previous experience with seniors preferred
Desire to serve and care for seniors
Ability to make resident-focused decisions
If applicable, driver duties require signing the Driver job description and safety understanding
