Overview RN Case Manager providing in-home nursing visits to clients who are former Nuclear Weapons Workers with chronic or terminal illnesses due to exposure to toxic substances during government work. Nursing visits are conducted weekly and typically last 1-2 hours per visit.
Responsibilities Perform in-home nursing visits, including physical assessments and vitals.
Ensure home safety with medical equipment and supplies.
Fill weekly medication planners and oversee skilled and non-skilled caregivers providing care in the client’s home.
Plan, implement, and evaluate care; participate in, coordinate, and manage client care conferences as needed.
Serve as a local on-site clinical resource and provide support to ensure client home care needs are met.
Collaborate with regional directors and staffing coordinators regarding staffing, scheduling, and scope of practice.
Adhere to nursing delegation guidelines and Agency policies.
Participate in interviewing, selection, and ongoing evaluation of clinical personnel as requested by the Regional Director.
Provide personnel training, education, and competency validation; review clinical documentation for accuracy and completeness.
Contribute to Agency performance improvement initiatives, including quarterly medical record reviews.
Collect, document, and submit data on infections, occurrences, complaints, grievances, and performance improvement activities.
Perform supervisory visits as needed to facilitate problem resolution and review nurse shift reports for policy adherence and improvement opportunities.
Ensure home chart completeness and timeliness of staffing cases post-referral; manage equipment tracking.
Assist with marketing activities (e.g., visiting with clients or physicians) as requested.
Senior case manager, or qualified alternate, remains available during operating hours for activities related to professional services and personnel assignments.
Perform additional duties as deemed necessary.
Qualifications Graduate of a state-approved school of professional registered nursing.
BSN preferred.
Current, unrestricted RN license in the state(s) of practice.
Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field.
One (1) year of supervisory and/or case management experience preferred.
Current CPR certification.

Standard Rate: $42.00 Hourly plus shift differentials, where applicable.
