Overview
Southern Jersey Family Medical Centers, Inc. is looking for a talented Chronic Disease Care Coordinator to provide care and help make an impact with our patients.
Job Level: Entry Level (less than 2 years)
Hours
This is a full-time position that requires the ability to work a flexible schedule split between the Burlington City and Buttonwood locations. General daytime hours Monday – Friday with at least one late night per week.
Responsibilities
Assist in coordinating patient care for visits, ordered tests, and other healthcare needs
Collaborate with providers and nursing staff to identify concerns that would prevent quality medical care
Assist with patients enrolled in case management
Provide effective and productive communication between patients and providers
Educate and assist patients in the management, prevention, and maintenance of their health wellness state through the use of evidence based guidelines and best practices
Assess and identify actual and potential patient problems by gathering relevant data
Address gaps in care of patients as identified in the electronic health record
Work with physicians and nursing staff to develop plan of care for patients and make adjustments as needed
Build and maintain relationships with both internal and external healthcare contacts and organizations in an effort to improve available patient resources
Track patient outcomes and provide information to authorized personnel as requested
Maintain electronic medical records (EMR) and other company information and ensure compliance with all State and Federal guidelines
Experience and Skills
1 - 2 years of experience working within a primary or ambulatory care setting
LPN, Social Worker, RMA or CMA preferred
Case management experience preferred
Excellent verbal and written communication skills and organizational skills
Ability to educate, motivate and inspire
Valid driver’s license
Computer skills
Bachelor's degree
