Southern Jersey Family Medical Centers, Inc. is looking for atalented Women’s Health Care Coordinator to provide care and help makean impact in our fast-paced facility.
Summary: The basic function of the Women’s HealthCoordinator is to provide patients with continuous, coordinated andcomprehensive health services to meet that individual’s needs bycoordinating Health Education with combined services from the nursingstaff, Nutritionist, Obstetricians and the Social Worker.
Hours: This is a full-time position that requires theability to work a flexible schedule. The position is based out ofBurlington City but will provide coverage to the Pemberton site forone day out of the week. General daytime hours Monday - Friday with 1to 2 evenings per week.
Position Responsibilities/Duties:
Orients obstetrical patients upon the first registration visitregarding the content and process of comprehensive maternity care services
Informs the patient of what services will be provided, who willprovide these services, where to obtain these services, when to gofor each given service, and who to contact in the event ofcomplications arising from these services
Arranges all patient services and linkages with appropriate agencies
Develops an individual care plan for each patient
Implements case conferences when necessary, with other careproviders, to implement the care plan
Performs all record keeping duties related to maternity careservices and chronic case management services
Provides, coordinates, reinforces and supports all healthteachings with patients as needed
Obtains all information regarding labor, delivery and post-partumrecords within two weeks of delivery and coordinates all post-partum visits
Provides coverage to all OB patients as needed
Perform other tasks as assigned by the Director of ClinicalSupport Services
Experience and Skills: At least 2 years of related work experience in a medical setting
Working knowledge of electronic medical records and referrals
Knowledge of medical terminology and office procedures
Excellent verbal and written communication skills
Bachelor’s Degree required, Healthcare/Social services,Psychology, Social Work, CHES or Public Health
