Overview Care Coordinator, Bureau of Tuberculosis Control
Join to apply for the Care Coordinator, Bureau of Tuberculosis Control role at NYC Department of Health and Mental Hygiene .
Responsibilities Review and enter data in the Health Department's TB surveillance and case management system.
Plan and conduct TB contact investigations in residential and non-residential congregate settings (e.g., worksites, schools, shelters).
Communicate with patients, health care providers, laboratories, and others (in person or on the phone) to conduct interviews, collect relevant information, and arrange for follow-up medical evaluation and treatment.
Assess results of investigations to determine if further expansion or public health intervention is warranted.
Assist in conducting TB education sessions at sites where TB exposures have occurred.
Conduct contact identification, notification, education, and care coordination.
Support administration and implementation of TB testing events in community settings.
Conduct TB testing, including specimen collection (phlebotomy) and specimen transport as needed.
Conduct medical chart review.
Other duties as required.
Qualifications A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
Education and/or experience which is equivalent to the above. All candidates must have at least one year of experience as described in the first option.
Preferred Skills Knowledge of the health care delivery system.
Experience working and communicating with patients and/or the public.
Experience working and communicating with clinicians or other health care or social service providers.
Experience with structured interviews, medical terminology, and conducting medical chart reviews.
Experience with public health education.
Good written and oral communication skills.
Excellent organizational skills.
Ability to work in a team environment.
Working knowledge of Microsoft Suite (Word, Excel, etc.).
Experience working in the community in a social service, mental health or public health capacity.
Must be willing to work occasional evenings and weekends.
