Job Summary Care Coordinator (CC) is part of a multidisciplinary team who serves as the bridge between behavioral health services, the criminal justice system, and human services and many other resources in the Bismarck regional area. As a care coordinator you will have regular meetings with individuals involved in the Free Through Recovery and Community Connect program. The bottom line of a Care Coordinator is to ensure clients obtain peer support once per week, conduct monthly care plans and meet monthly outcomes. This involves connecting clients to medical health, mental health and/or substance abuse treatment, human services, parole and probation, housing, employment, legal, educational, peer support and all other essential community-based needs and ensure follow-through has taken place for clients. This position focuses on serving the Bismarck region which includes Burleigh, Morton and Sioux counties.
Responsibilities Coordinate care for individuals across behavioral health, criminal justice, and human services networks in the Bismarck region.
Facilitate regular meetings with clients, ensure weekly peer support, and develop monthly care plans and outcomes.
Connect clients to medical health, mental health, substance abuse treatment, housing, employment, legal, educational, and other essential services.
Monitor follow-through and maintain documentation in accordance with program guidelines.
Required Qualifications At least an associate degree in human services or criminal justice with 2 years of related experience; other associate degrees will be considered.
Valid driver license and the ability to pass a background check.
Ability to maintain confidentiality and excellent written and verbal communication skills.
Strong attention to detail with time management and organizational skills for case management and documentation.
Cultural competence in working with Indigenous people with mental health, addiction, criminal records, or disabilities.
Ability to maintain professional relationships and be a team player.
Understanding of resources and services in the Bismarck-Mandan community.
bachelor’s degree in human services or criminal justice is preferred with some experience.
Preferred Qualifications Bachelor of Applied Science degree in behavioral health, human services, or criminal justice or another related field; other bachelor’s degrees will be considered.
Previously state certified in Care Coordination.
Prior case management experience with individuals involved in the criminal justice system, addiction, and mental health and/or disabilities with positive references.
