We are currently hiring a Care Coordinator - Amsterdam , to work in our Care Coordination Services Agency. We are looking for someone who enjoys working in a fast-paced team environment. This position is for someone who is a problem solver and enjoys helping those in need. As the Care Coordinator , you would be responsible for:
Assessing and developing care plans to assist clients in addressing gaps in care and social determinants of health
Coordinating the care of those with two or more chronic illnesses
Referring and linking clients to needed services and resources
Working with high-risk populations including clients with a mental health diagnosis, homelessness, HIV/AIDS, Sickle Cell Anemia, and/or Substance Use Disorder
Ability to work in a fast-paced environment
Care Coordinator - Amsterdam Minimum Salary: $19.23 hourly
Care Coordinator - Amsterdam Maximum Salary: $21.02 hourly
Care Coordinator - Amsterdam Schedule: Monday-Friday, 8:30am-4:30pm
Qualifications:
Bachelor’s Degree (preferred but not required) in Health, Education, or Human Services
1-3 years of case management experience
Experience working with diverse populations
