Job Summary
The Care Coordinator is responsible to assist with patient needs. Assist with managing care and addressing social determinants of health for Medicaid recipients with chronic health conditions.
Responsibilities
Build and maintain relationships with patients
Conduct face-to-face assessments for all patients to assess their medical and social needs
Create a care plan in adherence with providers and caregivers
Provide community resources to patients to ensure health and well-being
Promote timely access to appropriate care
Increase utilization of preventative care
Schedule appointments and transportation
Serve as the contact point, advocate, and informational resource for patients, the care team, family/caregiver(s), and community resources
Facilitate patient access to appropriate medical and specialty providers
Educate and refer patients to community resources
Keep detailed up-to-date documentation
Qualifications
2 years of experience in the medical field
Case Management or Care Coordinator experience preferred
Bachelor's degree required
Associate's degree acceptable with relevant healthcare or social services experience
