Overview The Care Coordinator is responsible for providing a variety of direct and indirect social services to clients and their families to facilitate their adjustment to and integration into the community.

Qualifications BSW or BA/BS in related field.
One year related Case Management experience.
Car and valid NYS driver\'s license required.
Ability to coordinate care amongst mental health, substance abuse and medical providers.
Ability to coordinate care for SMI population, along with health home clients diagnosed with chronic conditions.
Computer and software literacy including Microsoft Office.
Compensation Pay: 1199 Collective Bargaining Rates
