This is a full-time, work-from-home position. You must be located in the Indianapolis, Indiana area due to regular local travel (60%) for in-person patient appointments. The role requires fluency in Spanish.

You will work with treating physicians/providers, employers, customers, and injured/disabled individuals to restore health and return to work while ensuring cost-effective healthcare services.
Compensation information: the expected base pay ranges from $70,000 - $80,000 annually, with eligibility for a productivity-based bonus. Benefits details are provided in the Benefits section.

Responsibilities Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
Apply advanced knowledge of injury types, medications, comorbidities, treatment options, and job duties to advise on treatment plans.
Interview injured individuals to assess recovery and determine whether injuries are occupational or non-occupational.
Collaborate with treating physicians/providers and utilize available resources to create and implement individualized treatment plans.
Collaborate with employers and physicians to modify job duties to facilitate early return to work when practical.
Evaluate and adjust case goals based on patient progress and treatment effectiveness.
Independently manage workload, prioritize cases, and determine effective case management strategies.
Attend injured worker appointments when appropriate and prepare status updates for customers; perform other duties as assigned.

Qualifications Education: Associate’s Degree or Bachelor’s Degree in Nursing or related field.
Experience: 2+ years of clinical practice preferred; workers’ compensation experience preferred.
Skills: Ability to advocate effectively with physicians/providers, employers, and customers; ability to work independently; basic computer skills (Excel, Word, Outlook); strong grammar and written communication.
Certifications, Licenses, Registrations: Active Registered Nurse (RN) license in good standing. URAC-recognized case management certifications (e.g., CCM, CDMS, CRC, CRRN, COHN/COHN-S, RN-BC, ACM, CMAC, CMC).
Travel: Reliable transportation and ability to travel to in-person appointments within assigned geography.
Internet: Reliable internet access.
