BH Care Coordinator
This position reports directly to the BH Care Coordinator Supervisor and will collaborate closely with an assigned department within the BH division, which may change based on program needs. The role involves providing care coordination and outpatient behavioral health services to individuals across the entire LifePath system of care.

Responsibilities

Maintain a caseload and coordinate outpatient behavioral health services for individuals across LifePath.

Ensure timely, quality access to care and reduce barriers by troubleshooting and organizing support, interventions, and services.

Facilitate seamless transitions for individuals moving across health and social services; provide internal and external referrals as needed.

Manage documentation: sign service notes within two business days and conduct chart reviews with a score of 80 or higher.

Comply with the Health and Human Services Commission, Texas Administrative Code, SAMHSA grant requirements, and Lifepath Systems policies and procedures.

Collaborate with Adult Fidelity managers to reconcile all gaps in care.

Build and maintain professional relationships with community agencies, stakeholders, peers, leadership, and support teams.

De‑escalate behavioral health crisis situations calmly and respectfully.

Meet performance targets such as obtaining consent to project participation, timely admissions, NOMS reassessments, patient satisfaction surveys, regular contacts, and connections to CM, prescriber, and SUD services.

Practice good stewardship of resources by completing required trainings on time and maintaining center‑issued equipment.

Qualifications

Bachelor’s degree in psychology, social work, or a related field from a four‑year college or university.

Knowledge and experience with CCBHC.

Experience with Texas Recovery and Resiliency (TRR).

Knowledge of Trauma‑Informed Care Practices.

Valid driver’s license and reliable transportation.

Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention, and symptom management for individuals with serious mental illness or emotional disturbance.

Ability to assess needs, coordinate services, and communicate effectively.

Knowledge of the social services system and ability to connect individuals with appropriate services.

Strong written communication skills for documenting services and internal communication.

Capacity to think clearly and logically while handling potentially dangerous or emotional situations.

Patience, empathy, and understanding toward clients.

Organized, detail‑oriented, and computer‑proficient.

Demonstrated strong multicultural competency.

Bilingual (English/Spanish) preferred.
