Job Responsibilities

Collaborates with physicians and other medical staff to screen pregnant and postpartum women for mental illness.

Contacts patients with positive screens for brief assessment and triage.

Enrolls patients in collaborative care program and tracks their progress through patient registry.

Provides brief psychotherapy to patients.

Runs support groups for patients.

Collaborates with physicians and staff regarding patient situations, including weekly case management meetings and ad‑hoc communication.

Documents in medical records according to established procedures and by verbal communications as appropriate to ensure continuity of care.

Refers patients with higher acuity of mental health illness to appropriate other services (linked perinatal psychiatry service and/or community programs and/or emergency services).

Assists with staff in‑services on patient/customer service issues as needed.

Participates, as appropriate, in professional, educational, and supportive seminars, events, and community efforts.

Provides assistance to patients in arranging needed care services at home.

Stays up to date with governmental, professional, and institutional regulations, legislation, policies, and standards of care in social work in general. Stays up to date with changes in social work practice and social work/psychosocial literature.

Experience
At least 3-5 years of related experience. Experience with the perinatal population is strongly desired but not required.

Knowledge, Skills, and Abilities

Excellent organizational and time‑management abilities, able to work effectively within a multidisciplinary team.

Skilled with patient interaction, problem solving, patient navigation, listening.

Licenses and Certifications

Licensed clinical social worker in the State of New York.

Possesses the ability to facilitate the flow of information through effective written and oral communications.

Maintains a flexible and forward‑thinking approach to the way work is done.
