This position is located at our Medford Disability Office. Step into a pivotal role that combines compassion with expertise, as you facilitate Medicaid eligibility and coordinate essential services, enhancing the quality of life for individuals requiring long‑term care support.
Key Responsibilities
Screen and assess applicants, recipients, relatives, and other interested persons to determine initial and ongoing eligibility for LTSS, SPPC, and OPI‑M programs.
Develop detailed, individualized service plans. Conduct reviews in various care settings using OR ACCESS, CA/PS assessment tool, and related technology. Provide ongoing case management by coordinating the provision of services according to rules, regulations, and individual case plans (including monthly waivered service contacts and quarterly risk assessments). Make community referrals for additional services and supports including Adult Protective Services.
Use the OR ACCESS computer system to input client information, document all aspects of the case, and utilize DHR, ONE, and MMIS to collect, verify, and enter accurate data for benefit issuance.
Minimum Qualifications
Either a bachelor’s degree in Behavioral Science, Social Science, or a closely related field;
OR a bachelor’s degree in any field AND ONE YEAR of human services related experience (including work with economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or inadequate housing);
OR an associate degree in a Behavioral Science, Social Science, or a closely related field AND TWO YEARS of human services related experience;
OR THREE YEARS of human services related experience.

Working Conditions
Office hours 8am‑5pm, Monday‑Friday, with physical exertion such as climbing stairs, walking on uneven terrain, transporting a laptop, and traveling to homes, hospitals, nursing homes, and other provider settings.
Potential contact with individuals who may be hostile or violent, as well as with dangerous animals.
Exposure to various contagious diseases (e.g., staph infections, lice, MRSA, COVID‑19).
Valid driver’s license and acceptable driving record required.
Background Checks
Criminal history and background check required for finalists; adverse findings may lead to disqualification.
U.S. work authorization required via I‑9 and E‑Verify; no visa sponsorship offered.
