Aftercare Case Manager – TFI Family Services
We are looking for a full‑time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.

What You'll Need

A bachelor's degree in human services or a related field.

A valid driver’s license.

Willingness to learn – we provide all necessary training.

A Day in the Life

Focus on risk management and help families maintain stability during the aftercare period.

Provide crisis intervention and collaborate with public and private agencies offering supportive services.

Maintain accurate documentation and reports.

Coordinate community and educational services for families.

Meet with families in person to support reunification and resolve concerns.

Average hourly rate: $26.43.
