Join to apply for the Care Manager (RN) role at Sunflower Health Plan . The role offers a hybrid remote schedule with up to 50% in‑person visits in Eastern Kansas. Work is Monday‑Friday, 8 am‑5 pm.

Position Purpose
Develop, assess, and facilitate complex care management activities for members with primary physical needs to provide high‑quality, cost‑effective healthcare outcomes. This includes creating personalized care plans and educating members and their families.

Responsibilities

Enroll members in case management and conduct in‑person visits as required.

Evaluate member needs, barriers, and social determinants of health to recommend best outcomes.

Develop and update ongoing care plans, coordinating with providers and community resources.

Coordinate care with family/caregivers and provider teams to ensure appropriate, person‑centered services.

Provide ongoing follow‑up and monitoring of member status.

Offer resource support for local services such as employment, housing, and independent living.

Facilitate access to needed care through telephonic, digital, or site outreach.

Document all member information and care activities to ensure compliance with regulators.

Educate members and families on disease processes, care gaps, and benefits.

Provide feedback to leadership on improvements to care and quality.

Perform other duties as assigned.

Qualifications
Requires a degree from an accredited nursing school or a bachelor’s degree in nursing and 2‑4 years of related experience.

License/Certification
RN – Registered Nurse – State licensure and/or Compact state licensure required.

Pay & Benefits
