Overview ACT Case Manager – Behavioral Health at Community Support Services - Summit County. This full-time position provides ongoing community psychiatric support treatment services for clients with serious and/or persistent mental illness co-occurring with substance use disorder. The Case Manager will train, assist, and support clients to receive wrap-around rehabilitation services in the community. ACT Teams work together in a multidisciplinary approach to service planning and client community adjustment.
Responsibilities Collaborate with the client and the Service Coordinator to develop treatment plans that include services required to achieve recovery goals, outlining availability, intensity, and duration of each service.
Implement programming per the treatment plan and participate in ongoing assessment and therapeutic engagement of ACT team clients.
Observe and document client progress in accordance with agency procedures.
Assist clients in developing daily living skills and in obtaining medically necessary services (laundry, cooking, cleaning, communication and social skills, recreation, grocery shopping, personal hygiene, self-care).
Monitor medications to ensure proper dosages are taken as prescribed.
Assist clients with budgeting and accessing financial entitlements; advocate for needed services.
Perform on-call duty and weekend coverage on a rotating basis.
Maintain relevant documentation.
Work full-time (40 hours per week) during normal business hours.
Qualifications High school diploma required; bachelor’s degree preferred.
Non-bachelor’s candidates must be certified as an Ohio Certified Peer Support Recovery Specialist or qualified mental health specialist (QMHS).
Minimum of three years’ experience with clients with severe/persistent mental illness and/or substance use disorders preferred.
LSW, LPC, LPN, CDCA, LCDCII, or LCDCIII preferred.
Valid Ohio driver’s license and insurability required.

Ability to develop positive relationships with individuals with mental illness, developmental disabilities, and substance abuse challenges.
Skill in assessing strengths, needs, abilities, and preferences.
Strong organizational and communication skills; ability to work in a team environment; proficiency in Microsoft Outlook; ability to manage time and meet deadlines; ability to advocate effectively.
Knowledge of community resources and ability to access them.
Ability to work a flexible, varied schedule.
