Job Summary
The purpose of the Wound Care Nurse position is to support the nursing department and facility‑wide directives through specialized focus on wound care. This job description is a general overview of essential functions.
Major Duties and Critical Tasks
Facilitate the operation of the facility Infection Prevention/Control Program and support continuous quality improvement.
Support the facility and medical staff, departments, and teams with guidance, training, technical support, celebrating successes and continued learning related to infection prevention.
Chair the infection Prevention/Control Committee and serve as a member of other committees as assigned.
Compile and submit data related to Infection Prevention.
Support facility Quality Improvement activities with data support, analysis, and internal consulting.
Monitor and maintain current knowledge of key facility accrediting and regulatory agencies indicators/standards.
Develop policies and procedures that support infection prevention and control.
Consult with CNO and LTC DON and non‑clinical managers to ensure infection prevention and control measures are followed.
Identify, investigate, and report health care associated infections among patients and personnel through ongoing surveillance.
Maintain logs with infection surveillance data.
Maintain current knowledge of CDC guidelines and recommendations for infection prevention/control.
Calculate infection rates using the CDC’s recommended denominator data, prepare, and present reports to the Infection Control Committee.
Provide Infection Prevention orientation and other in‑service programs related to infection prevention and control for the facility.
Collaborate with HR and provide direction regarding employee health issues such as exposures, vaccines, and communicable diseases.
Make recommendations regarding real or potential problems/improvement opportunities.
Collaborate and report to appropriate agencies/regulatory organizations.
Follow the standards set by the Association for Professionals in Infection Control and Epidemiology (APIC).
Essential Responsibilities
Perform wound assessment, reassessment, and management for all patients per policy.
Knowledge of the anatomy, physiology, and disorders of the gastrointestinal, genitourinary, and dermal systems.
Utilize current wound management theory when providing care to patients with acute and chronic wounds and follow the standards of the Organization of Wound Care Nurses (OWCN).
Demonstrate thorough knowledge of the staging system of wounds.
Develop skin care programs to prevent skin breakdown.
Develop and implement policies and procedures for patients with draining wounds, fistulae, and/or tubes; wound prevention, wound care, ostomy care, and incontinence.
Revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan effectiveness.
Perform treatments and provide services within the nursing scope of practice.
Formulate a teaching plan based on identified learning needs and evaluate effectiveness.
Know medications and their correct administration based on the patient and clinical condition.
Act as a resource person to physicians, nursing, and other facility staff; provide in‑services on wound prevention, wound care, ostomy care, and incontinence.
Documentation meets current standards and practices.
Integrate research findings into clinical practice.
Participate in professional societies; stay current with advances in wound care.
Serve as a liaison between physicians, patients, and manufacturers of clinical products.
Know available clinical products on the market.
Operate all equipment correctly and safely.
Coordinate and supervise patient care as necessary.
Wound Care Nurse Qualifications
Current Licensed Practical Nurse (LPN) license by the state.
Wound care certification desired.
Skilled nursing experience preferred.
Excellent assessment skills, self‑motivated, good customer service skills; ability to work well with wound care consultants and organization.
