Job Purpose:
The Women's Health Connection (WHC) Care Coordinator plays a vital role in ensuring eligible women receive timely breast and cervical cancer screenings. This position involves coordinating diagnostic services, navigating healthcare systems, and supporting members throughout their care journey. The Care Coordinator serves as an advocate, providing compassionate guidance and ensuring continuity of care for WHC members.

Minimum Qualifications

Bilingual (Spanish/English) - Required

Certified Community Health Worker (CHW) - Required

High School Diploma - Required

Associate's degree in Social Work, Public Health, Sociology, Psychology, or a related field - Preferred

3+ years of experience in:

Healthcare-related social work, or

Case management with special needs populations

Experience in a clinical office setting - Preferred

Proficiency in Microsoft Office Suite, data entry, and database management

Strong communication and customer service skills

Valid driver's license and the ability to drive when needed.

Must pass a State of Nevada background check

Key Responsibilities

Coordinate and schedule breast and cervical cancer screenings and follow-up care

Assist clients in navigating healthcare systems and accessing community resources

Maintain accurate and timely documentation in client databases

Provide culturally competent support and education to WHC members

Collaborate with clinical and administrative staff to ensure continuity of care

Advocate for clients' needs and help eliminate barriers to care

Participate in outreach events and community engagement activities

Maintain confidentiality and adhere to HIPAA regulations

Preferred Experience

Working with socioeconomically disadvantaged populations

Familiarity with public health programs and community-based services

Experience in health education or patient navigation

Work Environment & Physical Requirements

Moderate noise level in a climate-controlled office

Ability to:

Stand for 3-6 hours/day

Sit at a computer for 4-6 hours/day

Perform repetitive hand/wrist movements

Lift and carry up to 30 pounds consistently
