Weekend Float RN Case Manager, Full-Time (Sign-On Bonus Available)
Transitions LifeCare is seeking a dedicated Registered Nurse (RN) for a full-time Weekend RN Case Manager position. As an industry leader, we are committed to providing exceptional care to patients and families facing end-of-life or life-changing illness. The position is based in our 8-county service area, and the schedule is Friday-Sunday, 7:00am-7:30pm, with one major and two minor holidays or three minor holidays per year. Experienced RNs with at least 1 year of experience may be eligible for a sign-on bonus of up to $20,000, based on specific qualifications and needs.

Key Responsibilities
- Comprehensive Care: Plan, provide, supervise, and document safe, competent, and compassionate hospice care for patients under the supervision of the Interdisciplinary Group (IDG) team.
- Collaboration: Work collaboratively with the IDG team, including physicians, social workers, chaplains, and other internal and external partners, to develop and implement a patient-centered care plan.
- Patient Assessment & Care Plan Management: Utilize professional nursing judgment to assess patients' needs and adjust care plans as necessary to ensure comfort, privacy, and dignity.
- Regulatory Compliance: Stay informed of federal, state, and company regulations related to hospice care, ensuring adherence to all guidelines.
- Education & Advocacy: Provide patients and families with the necessary information regarding end-of-life care, including symptom management, available services, and emotional support.
- Documentation: Ensure that all care provided is thoroughly documented according to regulatory standards, including Medicare and Medicaid compliance, and Transitions LifeCare policies and procedures.
Requirements
- Registered Nurse (RN) with current NC license; BSN preferred.
- A minimum of 1 year of experience as an RN; hospice and/or home health experience preferred.
- Must be able to travel to, enter, and function in patient homes/facilities within the service area.
- Must possess reliable transportation for home visits.
- Must pass all applicable pre-employment tests and meet annual training and state and federal guidelines.
