Position Summary

Provide case management and housing services to clients of Weaver House Emergency Shelter and Winter Emergency (WE) shelter programs. The role is full‑time, non‑exempt, hourly, reporting to the Director of Case Management. Work schedule is Monday‑Friday daytime with occasional evenings and a flexible arrangement to coordinate with client needs. Pay ranges from $20 to $22 per hour, based on education and experience.

Essential Functions

Provide ongoing case management, referrals, advocacy, budgeting, and other needed services.

Assist clients with obtaining permanent housing and addressing barriers.

Help clients develop skills for bill management and payment sustainability.

Engage clients using a client‑centered, strengths‑based approach and motivational interviewing.

Apply social work theories and evidence‑based models (Systems Theory, Solutions Focused, Cognitive Behavioral).

Assess mental health, substance abuse, and domestic violence issues; develop treatment plans.

Conduct group sessions for Weaver House clients using evidence‑based frameworks.

Complete VI‑SPDAT assessments as required.

Maintain documentation in files and the Homeless Management Information System (HMIS).

Refer residents to appropriate community resources.

Demonstrate commitment to Weaver House mission and vision.

Other Functions

Attend agency and program meetings.

Participate in weekly supervision.

Attend training related to employment functions.

Perform additional duties as directed by the Director of Case Management.

Remains flexible to management assignment changes.

Equipment Used

Telephone, copier, computer, printer, fax machine.

Working Conditions

General office environment.

Requirements

A bachelor's degree in social work, psychology, or a related social services field.

Preferred experience in human services.

Three (3) years of case management experience is preferred.

Other Requirements

Physical and cognitive abilities to speak, stand, sit, walk, read, write, type, organize, solve problems, plan, interpret data, make decisions, and drive.

Strong organizational skills and attention to detail.

Ability to work with people with diverse needs.

Knowledge of community resources.

Confidentiality and professional boundaries.

Understanding of homelessness, Rapid Rehousing, and Housing First.
