SCHEDULE AND COMPENSATION:
Monday – Friday, daytime hours
Hourly Rate: $22.15-$27, dependent on experience/education
$22.15 - Very little-to-no experience in human services
$23.50 - Experienced waiver case manager or experienced human services professional
$26 - 5+ years of experience as waiver case manager or 10+ years as a human services professional

SOME IMPORTANT QUALIFICATIONS:
To be eligible to provide AC/BI/CADI/EW Case Management, you must have: Bachelor’s degree in health and human services OR a bachelor’s degree in any field with 1-year of experience in the human services field
To be eligible to provide DD Case Management, you must have: Bachelor's degree in social work, Special Education, Psychology, Nursing, Human Services or related fields AND
One year of direct experience working with individuals with developmental disabilities or related conditions OR have taken a course focused on developmental disabilities
Ability to pass DHS background check
Driver’s license & vehicle insurance
MORE ON THE ROLE: As a Waiver Case Manager , you'll be part of a team based out of our headquarters in Saint Paul. Responsibilities include:
Complete new client intakes
Manage a caseload of around 45 clients
Develop initial service plans with clients and legal representatives
Coordinate services between providers and clients
Schedule in-person meetings at least twice per year and maintain monthly phone contact
Maintain quality case notes documenting service plans, progress, and agreements
You will work to uncover client needs and help remove barriers by connecting them with services and resources, acting as a liaison to ensure their care team is aligned and supportive of their goals.

CADI - Community Access for Disability Inclusion Waiver
DD – Developmental Disabilities Waiver, Rule 185, VA/DD
EW – Elderly Waiver
As a Waiver Case Management Supervisor , you will manage a small team of Case Managers while maintaining a partial caseload.
