About the Job
As a Case Manager MSW on the VASH Team at the RHC Office, you will provide field‑based, direct client outreach, linking, and retention services to people experiencing homelessness. Your Services include individual and group therapy, outreach, case assessment, service planning, assistance with subsidy applications, housing location, advocacy, and addressing health, mental health, and substance use barriers.
Position Responsibilities
Perform initial comprehensive biopsychosocial assessment and develop a plan of intervention.
Collaborate with clients to develop individualized treatment plans with short and long‑term goals.
Provide ongoing case management support, assessing progress and ensuring treatment plan outcomes are met or adjusted.
Deliver field‑based therapeutic supportive case management addressing mental health, functional, and substance use barriers to help clients obtain and maintain housing.
What You Bring
Master’s Degree in Social Work or equivalent behavioral health degree eligible for licensure through the California Board of Behavioral Sciences.
One year of experience in healthcare, case management, homeless services or working with vulnerable populations.
Analytical skills, communication, collaboration, critical thinking, ethics and boundary maintenance, creativity, and initiative.
Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW) is preferred.
Mandatory Requirements
Employment eligibility verification.
CPR/First Aid training or ability and willingness to obtain.
Completion of Tuberculosis Test, Background Screening, and Drug Test as condition of hire.
