Overview Triage Nurse - Remote (South Carolina resident) at SouthernMED Pediatrics. This role provides clinical assessment, patient education, and care coordination via telephone and electronic communication for an outpatient pediatric practice. The position supports families by assessing symptoms, determining the appropriate level of care, providing home care instructions, and ensuring timely communication with providers.
Responsibilities Answer incoming clinical calls and electronic messages from patients and caregivers in a timely, professional manner.
Conduct telephone assessments using established pediatric triage protocols.
Provide appropriate advice for home care or escalation to in-office or emergency care as needed.
Document all encounters accurately and thoroughly in the electronic health record (EHR).
Communicate with physicians, nurse practitioners, and other care team members regarding patient concerns requiring provider input or follow-up.
Manage refill requests, lab results, and other clinical communications as directed.
Assist with care coordination for complex patients as needed.
Educate families about symptoms, medications, and follow-up care.
Support the overall efficiency and quality of patient care by assisting with clinical workflows as appropriate.
Meets position requirements and performs essential functions.
Completes all mandatory training as required.
Performs other duties as assigned.
Qualifications Registered Nurse (RN) or Licensed Practical Nurse (LPN) with an active state license required.
Minimum 3–5 years of pediatric nursing experience, including at least 2 years of triage and/or leadership experience in an outpatient or primary care setting.
Strong clinical judgment and knowledge of pediatric triage protocols.
Excellent communication and customer service skills for interacting with families and staff.
Proficiency with electronic health records and telephone triage systems.
Leadership abilities including team management, coaching, and conflict resolution.
Prior experience with EMR preferred.
Demonstrates proper telephone etiquette.
Ability to work independently without direct supervision.
Ability to coordinate, analyze, observe, make decisions, and meet deadlines, in a detail-oriented manner.
