Treatment Apartment Case Manager

Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role provides person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability. You’ll help participants build life skills, access housing and behavioral health services, and foster community and family connections while promoting wellness and stability.
Responsibilities Implement individualized treatment and rehabilitation plans.
Provide consultation and foster peer support among participants.
Ensure safety and upkeep of residences, equipment, and supplies.
Assist individuals in securing and maintaining stable housing.
Conduct assessments and develop service plans.
Coordinate tenancy support, including conflict resolution and benefit navigation.
Maintain accurate documentation and comply with agency and regulatory standards.
Provide crisis support in various community settings.
Work closely with treatment teams, families, and external partners.
Build and maintain relationships with landlords and community providers.
Support internal and external reporting requirements.
Qualifications Bachelor’s degree in a human-services field, OR
Associate’s degree + 3 years of experience, OR
High school diploma + 5 years of experience in human services.
Minimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred.
Prior experience in OMH Housing Programs strongly preferred.
Valid NYS Driver’s License and reliable transportation.
This position typically follows a Monday–Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs.
