Overview Travel Assignment RN - Case Manager - $64.15/hr 13wk
Responsibilities Average census and case load/day – 99% capacity on most days; 14-16 patients – dependent on unit assigned
Care management skills: assessment, care coordination and discharge planning
Skilled in placement for SNF/IPR/LTAC/Hospice/Home Health, DME, Behavioral Health needs/DSS (peds); difficult complex placement; routinely work with homeless population
Required EPIC experience; EPIC Roster (Rover Chat) and Careport referral management (preferred experience)
Possible to float to any unit; orientation will vary by skillset/comfort level (2-5 days)
Scheduling: weekend rotation depending on requisition; no call; holiday work required unless contracted for time off
Scrub color: Navy Blue; parking: Free
Qualifications RN: BSN required; BLS; Case Management certification (preferred)
Minimum 3-5 years in Case Management
First-time traveler: No; must have traveled as an RN-Case Manager
Required licenses and certifications as applicable (Driver's License and/or Insurance may be required depending on position)
Adult Medical/Surgical, Cardiac, Orthopedics and possibly Pediatrics depending on unit
EPIC experience required

457 licensed beds; Trauma Level III; Teaching Hospital
Chest Pain Center; Stroke Center of Excellence; Bariatric Center of Excellence
Department culture: supportive with collaborative teams (UM, CM, social workers, educators)

Floating: possible to any unit
Rover Chat in EPIC and phone duties
Department education and changes communicated by managers
