Position Title: Transitional Living Case Manager
Responsible to: Assistant Executive Director
Summation of Job Responsibilities:
1. Assist the Leadership Team in the delivery of all services to the clients of the program.
2. Establish a schedule for TLP clients to help them achieve their goals and reach self-sufficiency.
3. Coordinate required educational classes for the TLP. Facilitate classes as needed. These will include, but not limited to: fetal development, prenatal self-care, labor and birth, postpartum/postnatal self-care, stress management, nutrition and exercise, career and finance, infant care, co-parenting, partner/friend/family support during pregnancy, responsible paternity education, local resources available, and discharge planning.
4. Arrange or provide transportation for clients to school, work, and appointments, as needed.
5. Arrange and assist in the move-in of each family to the TLP, including but not limited to a referral for apartment furnishings and the loading and unloading of client belongings.
6. Attend all important professional meetings and appointments with clients, as required.
7. Manage and oversee the client’s personal budget, savings, and rent payments. Review pay stubs with clients to verify income.
8. Assist in the completion of potential client interviews, intake assessments, key performance indicators (KPIs), and make referrals as needed.
9. Assist clients in the process of applying for support services, such as financial assistance, food stamps, and medical coverage by referring and accompanying clients to the appropriate agencies.
10. Maintain all client records, case notes, extendedReach documentation, and communication with any professional teams.
11. Document any client interaction that may need follow-up with the leadership team (ie. behaviors, unmet goals and expectations, and program disengagement).
12. Complete three weekly check-ins with clients, with at least one in-person meeting. Complete weekly (at minimum) apartment checks for each TLP family.
13. Establish and monitor progress towards clients’ Individualized Treatment Plan, weekly SMART goals and budget. Provide hands-on support and advocacy as needed.
14. Utilize incentives for client motivation and program engagement, as needed and appropriate.
15. Complete “Project Attach” curriculum and assessments with all clients in the TLP.
16. Assume the duties of other staff members as the result of vacation schedules, illness or absences as necessary and assigned by the Programs Coordinator.
17. Perform all other tasks as assigned by the Programs Coordinator.
Qualifications for the Position:
1.Education: Minimum of a Bachelor’s Degree in Psychology, Social Work, Counseling or a related human services field.
2. Experience: At least 3 years of experience in at least 1 of the following: early childhood development, family and marital counseling, social work, or case management.
3. Knowledge and experience working with teenagers, single parents, homeless and care of newborns is highly preferred.
4. Ability to multi-task, think critically, and problem-solve proactively with clients.
Requirements for the Job:
Character: Good interpersonal skills, reputable character, values, and ethical standards conducive to the well being of the population.
Must be able to pass a Child Abuse/Neglect background check and federal fingerprints.
Must provide agency with three positive written references.
Must be at least 21 years of age.
Must possess a valid driver license with an acceptable driving record and must have current car insurance.
Requirements for the Job:
Hours: Assigned Shifts, 40 Hours per week. On-Call as needed.
At least 50% of time should be spent on field work with clients, and up to 50% of time can be spent in office completing documentation.
Must be able to climb stairs, use limbs to lift at least 50 pounds, and care for infants, toddlers, and clients without assistance.
Dress Code: Conservative casual clothing is acceptable in the office and shelter. Dress clothing should be worn while attending meetings, community functions and representing Mother’s Refuge to the public.
