Overview Performs moderately complex (journey-level) to advanced and/or supervisory (senior-level) case management work to develop and maintain long-term contact with assigned youth, their families, and providers for medical, social, educational, and related service needs. Work involves completing case file reviews, case planning, family involvement and group service delivery, developing goals, and evaluating outcomes. Performs discharge and release functions to ensure coordination of service delivery throughout the youth's commitment in TJJD. May work in excess of 40 hours per week, with different shift patterns and on-call requirements.
Responsibilities Conducts and completes initial case assessments and file reviews, requests missing documentation as needed, performs stage assessments, and conducts ongoing assessments for the assigned caseload.
Completes Case Planning for the assigned caseload, applying appropriate strategies to facilitate youth progress and make specialized referrals (e.g. additional assessments, MHTP/MROP referrals, TCOOMMI, etc.).
Monitors caseload to complete transition planning and facilitates required groups for the caseload; plans for scheduled and unanticipated leave of absence to ensure provision of group services by an approved alternate group leader.
Participates in staff development and trainings related to treatment programs, CMS, and case management standards; provides individual counseling to the youth and family contacts.
Completes documentation of services, data entry, and maintains case sub-files; reviews youth health, safety, and facility-related information and shares relevant details with staff.
Complies with policies, procedures, rules, and regulations; prepares and maintains records, forms, and reports; performs other duties as assigned and ensures safety policies are followed.
Attends work regularly and on schedule in accordance with agency attendance policies.
Qualifications MINIMUM QUALIFICATIONS
Requirements for Case Manager II - Pay Level One (1): Unrelated Bachelor’s degree. No experience required. OR High School diploma or equivalent, and Fifteen (15) semester hours of college coursework, and One (1) year full-time wage-earning experience in case management work or related responsibilities in a residential setting for youth, including assessments, development and implementation of individual case plans, service coordination, and referrals; or related experience in parole, social work, or TJJD/TYC roles. Senior/Levels and Career Path Case Manager II: Moderately complex journey-level work under general supervision with some independence.
Case Manager III: Highly complex senior-level work under limited supervision.
Case Manager IV: Advanced and/or supervisory senior-level work under minimal supervision.
