Overview Join to apply for the TASC Care Manager (Windsor, NC) role at Coastal Horizons .
Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to support justice-involved individuals by providing case management and service coordination within the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services. This opportunity is located in Bertie, Chowan, Perquimans, and Currituck Counties with Windsor, NC as the primary office.
Location: Bertie, Chowan, Perquimans, and Currituck Counties (Windsor, NC)
Hours: 40 hours/week, Full Time, M-F, Non-Exempt

Responsibilities Primary duties include administering psycho-social screenings/assessments.
Coordinate linkage to behavioral health treatment as well as medical, educational, and vocational services as needed.
Provide ongoing professional care management services for the justice-involved population.

Qualifications Education: High school diploma or higher.
Certifications: Preference given to NC Peer Support Specialist certification and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
Experience: Knowledge of recovery, harm reduction, and wellness-based concepts. Lived experience with recovery and/or the justice system is a plus.

Required Skills Strong communication, organizational, and time-management skills.
Ability to work independently and manage a caseload effectively.
Proficiency in using electronic health record systems and remote collaboration tools (e.g., Teams, Zoom).
Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.
