Job Title: TASC Care Manager
Location: Craven County (New Bern, NC) & Jones County (Trenton)
Hours: 40 hours/week, Full Time, M-F, Non-Exempt
Job Description: Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team. This position offers a unique opportunity to support justice-involved individuals by providing case management and service coordination within the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services. Primary duties include administering psycho-social screenings/assessments, coordinating linkage to behavioral health treatment as well as medical, educational, and vocational services as needed, and providing ongoing professional care management services for the justice-involved population.
Locations: Craven County (New Bern) and Jones County (Trenton)
Qualifications: High school diploma or higher
Preference given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
Knowledge of recovery, harm reduction, and wellness-based concepts. Lived experience with recovery and/or the justice system is a plus.
Required Skills: Strong communication, organizational, and time-management skills.
Ability to work independently and manage a caseload effectively.
Proficiency in electronic health record systems and remote collaboration tools (e.g., Teams, Zoom).
Ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.
Salary & Benefits: Competitive salary range: $42,000-$45,000, depending on experience
