Target Case Manager
Job Summary The Target Case Manager secures, coordinates, monitors, links and advocates for unified goals and services with organizations and personnel on behalf of the identified patient and their family, in targeted populations, in order to meet their individual needs and achieve successful outcomes.
Duties and Responsibilities
Works with community partners and available resources, attempting to minimize barriers that prevent the identified patient and family from accessing services
Gathers and relays clinical information to secure funding for mental health services for patients identified as needing a higher level of care due to emotional or behavioral difficulties and substance abuse, frequently classified as SMI, SED or SUD
Collaboratively develops goals and works on identified treatment needs in different domains of functioning: Basic Care, Educational, Vocational, Physical Health, Legal, and Mental Health
Adheres to Cultivate policies and procedures
Performs other duties as assigned
Qualifications
Bachelor's degree in a helping field
One year of experience working with the targeted populations
Knowledge, Skills And Abilities
Process and deadline driven, keen attention to detail
Highly motivated and energetic during work hours while maintaining a record of reliable attendance and punctuality
Ability to deliver timely results while managing multiple projects with competing priorities
Excellent interpersonal skills
Sophisticated communication skills including actively listening
Ability to work independently or as part of a team
Ability to form and maintain relationships and to model and encourage positive collaborative behaviors
Proficient in the use of the Microsoft Office Suite, including Word, Excel, PowerPoint, Outlook eMail and Calendar
Ability to successfully complete required background and drug screens
