Requirements

Must have a Bachelor’s degree in Social Work, Counseling, or a related field from an accredited institution

Minimum of one (1) year clinical experience working with children and families

Valid Driver's License

Proficiency with Microsoft Windows and Office applications

Strong documentation and time management skills

Ability to work independently and collaboratively

Job Summary
The primary role of this position is to deliver community-based behavioral health services tailored to the mental health needs of children and families. This includes performing a variety of functions across multiple settings, while considering family dynamics, cultural preferences, and community environments. The individual in this position will provide direct client services, engage in internal and external collaboration, assist in crisis management, and support the development of individualized service plans. They will also ensure continuity of care and provide administrative support, all while adhering to regulatory and agency standards.

Key Responsibilities

Provide behavioral health interventions, including needs assessment, crisis management, and skill development.

Develop and coordinate Individualized Service Plans (ISPs) in collaboration with clients, families, and service providers.

Facilitate team meetings and coordinate treatment with other providers.

Assist clients in accessing community resources and support systems.

Promote skill generalization in community settings and support transitions between care systems.

Prepare meeting agendas, take notes, and ensure timely documentation.

Adhere to agency standards and participate in professional development.

Provide interventions based on best practices and engage in process improvement activities.

Compensation & Benefits

$18.00 to $21.00 an hour
