Hendersonville, NC and surrounding area.
Remote work from home, with travel to clients' homes.
Flexible hours, including evenings, weekends, holidays.
Available 24/7 to clients on caseload.
Travel to various facilities (human services agencies, clinics, hospitals, etc.).

Deliver Tailored Plan Care Management to children with behavioral health needs and their families, supporting holistic wellness.

Hiring Range
$49,000 to $55,000 annually (based on 40‑hour work week).

Education / Experience / Licensure
Bachelor’s degree in health, psychology, sociology, social work, nursing, or related field, or RN license.
Qualified Mental Health Professional credential: Master’s with 1 yr supervised clinical experience OR Bachelor’s with 2 yr experience OR Bachelor’s non‑human services with 4 yr experience.
Minimum 2 yr experience working directly with individuals with behavioral health conditions.
Equivalent combination considered.

Knowledge of Child Welfare & Behavioral Health Systems.
Excellent interpersonal, communication, customer service, organizational skills.
Trauma‑informed care understanding.
Case management proficiency (high caseload, assessments, care plans).
Crisis management and adaptability.
Relationship building with families, partners, teams.
Computer skills (Microsoft Office, Google Suite).
Time management.
Creative, positive attitude.
Cultural competence.

Key Responsibilities
Oversee caseload of 30–129 clients (ages 3–21).
Complete comprehensive assessments.
Create individualized care plans.
Coordinate multidisciplinary team.
Provide 24/7 emergency support.
Monitor medication adherence.
Connect clients to community resources.
Educate on self‑care and preventive health.
Facilitate seamless transitions between care settings.
Support families in accessing services.
