Overview Join the Supportive Case Manager team at Southwest Network to help children and families thrive through the Child and Family Team (CFT) process.
Responsibilities Facilitate or assist in CFT meetings and conduct observations and assessments of youths using developmental and/or mental health assessment tools and trauma screenings.
Work in members’ homes and out in the community, maintaining regular contact with foster families, caregivers, group homes and shelters to assess the needs of assigned youths and assist youths/families in securing needed services.
Qualifications Education: Master’s or Bachelor’s degree in a behavioral health or related field, or equivalent experience and education per the table below:
Bachelor’s in a non-behavioral health field PLUS one year related experience
Associate’s degree PLUS one year related experience
High school diploma PLUS three years related experience
Certifications: Current CPR, First Aid, and Therapeutic Options certifications; Arizona state fingerprint clearance card; valid Arizona driver’s license and car insurance.
Minimum age: 21 years.
Knowledge, Ability, and Skills Excellent member and family relationship building and customer service skills.
Effective written, verbal, and interpersonal communication skills.
Strong organization, critical thinking, time management, and problem‑solving skills.
Proficiency using multiple office equipment (phones, copier, fax, etc.).
Typing speed of 25–40 WPM preferred; proficiency in Microsoft Office and internet use.
