Summary:
Under the direction of the Program Manager, Contingency Management, the Substance Use Case Manager will collaborate with members of the Contingency Management Pilot mobile team to provide support and engage participants enrolled in Project Based Housing sites. The pilot project supports participants to improve housing stability, health and other targeted behaviors through the use of case management services and incentives. The pilot is a collaborative effort of Department of Health Service's Harm Reduction Division and Housing for Health, the Department of Public Health's Substance Abuse Prevention and Control Harm Reduction team, and Brilliant Corners.

The Housing Retention Contingency Management mobile team consists of a Program Manager, a Peer Counselor, a Substance Use Counselor and a Nurse Practitioner. The team visits participants at their housing location, develops contingency management goals, assesses progress towards achieving those goals and issues incentives. The team will work closely with the participant's Housing for Health Intensive Case Management Services Case Manager and property or facility management (as appropriate) to support the participant's progress towards reducing problematic drug use and to retain housing for participants at-risk of losing their housing due to substance use related behaviors. Abstinence is not required to participate in the program.

Essential Duties and Responsibilities:
Completes all necessary enrollment procedures and documentation for Contingency Management program participants.
Provides orientation and guidance to participants, and coordinates with staff to ensure collaboration on Contingency Management services.
Provides care by delivering and tracking all Contingency Management services, including accomplishing goals and documenting progress.
Provides field based safer drug use supplies, sexual health supplies, and harm reduction education to homeless individuals.
Act as an expert on the Harm Reduction Model by furthering the equity of our participants through harm reduction services by utilizing a diversity, equity, and inclusion lens.
Provide connections to Medications for Addiction Treatment, and monitors participation.
Collaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability.
Develop and maintain a complete, accurate, and current client file with all required documents and data in agency records and electronic databases.
Document all client activity in accordance with funder expectations.
Coordinate with DHS and community partners to ensure clients are connected to primary health care and insurance to reduce need for emergency health care services
Maintain confidentiality of client files per HIPAA and all applicable guidelines
Link participants to detox and substance use recovery programs when appropriate.
Conducts substance use assessments.
Develops a plan to support participants in managing triggers and cravings.
May be required to drive the program vehicle (Sprinter van)
Other duties as assigned.

Job Description Qualifications
Certification in substance abuse counseling. (CAADE and/or CAADAC preferred).
Bachelor's degree in a related field or equivalent experience.
Minimum two years' experience as a substance abuse counselor and/or case manager, in a licensed and/or accredited substance abuse treatment facility (experience in a dual-diagnosis setting preferred).
Able to work in a high-tolerance Harm Reduction model with clients who have multiple barriers.
Computer literacy, including knowledge of Microsoft Word, Outlook, and Excel
Able to obtain and maintain CPR/1st Aid certification

Preferred Qualifications:
Knowledge of the principles and practices of harm reduction, culture, terminology, and literature
Bilingual in English/Spanish a plus

Work Environment
Combination of field (mobile team on van) and office environment
Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
On occasion, walk or drive to different local sites
Will necessitate working in busy and loud environments
Will be exposed to elements like cold, heat, dust, noise, and odor
May need to bend, stoop, twist, and sit throughout the day
