Overview
Stepping Up Case Manager role at AltaPointe Health Systems.

Responsibilities

Coordinates and provides services related to the “Stepping Up” program, working with the Alabama State “Stepping Up” Coordinator, Technical Assistance Team, and local community partners to reduce recidivism and re-entry of people with mental illness or co-occurring mental illness and substance use disorders into local county jails and hospitals.

Conducts case management services for participants in the “Stepping Up” program; implements case management plans, advocates for patients with community providers, links patients with community services, and monitors progress.

Represents the needs of individuals admitted to the “Stepping Up” program and ensures quality services; participates in trainings and meetings related to Stepping Up standards and alliances.

Completes needs assessments using required forms to determine assets, strengths, deficits, barriers, and needs.

Performs basic screenings for patient risks, substance use, functional deficits, and social determinants of health.

Develops case management service plans after assessing needs and consulting with referral sources; assists patients in setting goals related to areas such as grooming, skills teaching, medication knowledge, and financial resources.

Implements and monitors the case management plan; conducts home visits, follows up on referrals and medications, and consults with treatment teams on noncompliance; assists with transitions from hospitalization or incarceration to home or lower levels of care.

Establishes liaison with community resources, identifies service gaps, advocates for patients, and provides transportation as needed and appropriate.

Participates in quality assurance by completing required case management training and attending meetings and tasks as required.

Clinical Record Keeping

Documents in a timely, clear, and legible manner per AltaPointe policy.

Adheres to documentation standards in all records.

Respects patient privacy and confidentiality; is sensitive to patient needs and diversity.

Administrative and Other Duties

Participates in Performance Improvement activities and AltaPointe committees as required.

Completes assigned tasks in a timely manner and follows AltaPointe policies and procedures.

Attends in-services training and other workshops as appropriate.

Qualifications

Bachelor’s degree in a behavioral health, mental health, or social services related discipline.

Valid verifiable state-issued driver’s license.

Certification of case management training by the Alabama State Department of Mental Health and Mental Retardation.

Knowledge and Training

Knowledge in adult behavioral health and substance use disorders; Trauma-Informed Care principles; cultural diversity.

Knowledge of behavior management, crisis intervention, and non-violent crisis intervention practices; educational methods for adults with psychiatric disorders.

Knowledge of benefit entitlements (e.g., Social Security, Medicaid, Medicare), use of electronic health records, available community resources, and referral sources for physical health, mental health, social services, and substance abuse treatment.

Knowledge of screening tools for behavioral health and substance use.

Training to be Completed

Case Management training by the Alabama State Department of Mental Health.

SOAR training, Medicaid Assistor Training, Suicide Severity Rating, DLA-20, UNCOPE, CLIA training for UDS, Motivational Interviewing, Trauma Informed Care.
