Summary of Position
The Speech Pathologist evaluates and directly provides or supervises the provision of speech therapy services to patients in their homes or residences. When functioning in the capacity of the Home Care Case Manager (HCCM), the Speech Pathologist is responsible for the provision and coordination of all components of a patient’s home care services. The Speech Pathologist provides care as ordered by the physician, follows policies/procedures and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the development and implementation of the plan of care as outlined by the physician. Additionally, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.
Nature and Scope
The role of the Speech Pathologist/HCCM is to oversee the home care services provided to patients and, specifically, those services related to their field of practice. This position interfaces with all levels of home care staff as required by Medicare Conditions of Participation and TJC.
Qualifications
Education – Master’s degree in Speech Therapy/ Speech and Language Pathology or equivalent as determined by the state Board.
Licensure/Certification/Registration – Current Ohio Speech and Language Pathology License; CPR Certification.
Experience – Minimum 1–3 years of job‑related experience. Preferred: Home care experience.
Prior Clinical Experience – At least one year of clinical experience preferably in an acute‑care setting within the last three years unless currently employed in home care.
Additional Requirements – Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.; must creatively adapt multiple environments and situations for successful outcomes; must be self‑directed and able to work with little supervision; computer skills preferred.
